नाम, dose, allergy और doctor contact की छोटी सूची emergency में बहुत मददगार हो सकती है।

अस्पताल में सबसे पहले यही पूछा जाता है कि आप कौन सी दवाइयाँ लेते हैं। सूची पहले से लिखी हो तो उस समय किसी को याद करने की ज़रूरत नहीं पड़ती।The first thing a hospital asks is which medicines you take. If the list is already written down, nobody has to remember it at the worst possible moment.
अस्पताल या नए डॉक्टर के पास सबसे पहले यही पूछा जाता है कि आप कौन सी दवाइयाँ लेते हैं। उस समय याद करना कठिन होता है — और साथ गए व्यक्ति को अक्सर पता ही नहीं होता।

एक कागज़ पर्स या बटुए में, वहीं जहाँ पहचान पत्र रहता है।
उसी कागज़ की एक फ़ोटो फ़ोन में रख लीजिए।
दवाओं के पत्तों की भी एक फ़ोटो रख लीजिए — नाम पढ़ने में आसानी रहती है।
कब बदलें
जब भी डॉक्टर कोई दवा जोड़ें या बंद करें, उसी दिन सूची सुधार दीजिए। पुरानी सूची कभी-कभी सूची न होने से भी ज़्यादा भ्रम पैदा करती है।
यह सूची इलाज की सलाह नहीं है। इसका काम केवल इतना है कि ज़रूरत के समय सही जानकारी तुरंत सामने रहे।

When, and how many times a day
What it is for, if you know
Anything you are allergic to
Your doctor's name and number
The number of someone at home
Your blood group
Where to keep it
One sheet in your purse or wallet, with your identity card.
A photograph of that same sheet on your phone.
A photograph of the strips as well — the names are easier to read from them.

The day a doctor adds or stops anything, correct the list. An out-of-date list can confuse matters more than having none at all.
